Consent Terms & Conditions
This form is optional
It can only be completed by the Participant or their Account Nominee.
Complete this form only if the Participant wishes to give another person access to their information.
There are two types of consent available:
1. Account Nominee
An Account Nominee has full access and decision-making authority for the account. They can:
- Fully access the participant’s profile
- Make decisions regarding the account
- Update profile details
- Authorise the addition of other consented contacts to the profile
- Request copies of invoices
- Be nominated as the main profile contact
- Receive statements
- Access the client profile
- Approve invoices
2. General Consent
A General Consent contact has limited access and no decision-making powers. They can:
- Request budget and funding information
- Request details about processed invoices
- Receive general queries from our Plan Managers
- Be nominated as the main profile contact (if chosen by the participant)
- Receive statements
- Access the client profile (if permitted)
They cannot:
- Make decisions regarding the account
- Change profile details
Important Notes
- Only provide consent to someone you trust.
- If the participant is 18 or older, they will automatically be an Account Nominee.
- If the participant is 18 or older and should not have Account Nominee access, Linda Health Partners require legal documentation showing guardianship or relinquishment of decision-making rights.
- In case of a dispute between the participant and the Account Nominee regarding choice and control, Linda Health Partners will follow the participant’s wishes unless the above legal documentation is provided.
- No one can request account information from Linda Health Partners without consent.
- It is the participant’s and/or Account Nominee’s responsibility to notify Linda Health Partners to add or remove consent when required.
